Sir,

With due respect and humble submission, I beg to state that I am a regular student of class X/XI of your school/college. My mother is very ill. She has been suffering from cancer for many days. At present, her condition is very serious. The local doctors have advised us to take her to a cancer hospital. According to their advice, my father has decided to take her to the Dhaka Cancer Hospital. I have to go with my mother. So, I need five days' advance leave from the 6th to the 10th instant. On April 11, I will come to school/college again.

I, therefore, pray and hope that you would be kind enough to grant me advance leave for those days only and oblige thereby.

জনাব,

যথাযথ সম্মান এবং বিনয়ের সাথে আমি অনুরোধ করছি যে আমি আপনার স্কুল/কলেজের দশম/একাদশ শ্রেণীর একজন নিয়মিত ছাত্র। আমার মা খুব অসুস্থ। তিনি দীর্ঘদিন ধরে ক্যান্সারে ভুগিতেছে। বর্তমানে তার অবস্থা খুবই আশঙ্কাজনক। স্থানীয় চিকিৎসকরা তাকে ক্যান্সার হাসপাতালে নিয়ে যাওয়ার পরামর্শ দিয়েছেন। তাদের পরামর্শ অনুযায়ী আমার বাবা তাকে ঢাকার ক্যান্সার হাসপাতালে নেওয়ার সিদ্ধান্ত নিয়েছেন। আমাকে আমার মায়ের সাথে যেতে হবে। সুতরাং, আমার ৬ তারিখ থেকে ১০ তারিখ পর্যন্ত ৫ দিনের অগ্রিম ছুটি দরকার। ১১ই এপ্রিল, আমি আবার স্কুল/কলেজে আসব।

অতএব, আমি প্রার্থনা করি এবং আশা করি যে আপনি আমাকে ঐ দিনগুলির জন্য অগ্রিম ছুটি দিতে যথেষ্ট সদয় হয়ে আমাকে বাধিত করবেন।
